Terms and Conditions for Providing Indemnity
The insuree (the cardholder, or a third-party) is obliged to report insurance events by telephone to the telephone number that the insuree was given of when receiving the payment card and which is given in the additional materials for the payment card (Call Centre), or in person at any CSOB branch.
The insuree is obliged to submit the "Notification of an Insurance Event" form within no more than two months of their having reported the insurance event to the insurer. All paragraphs of the form must be filled out and signed by the insuree.
In the event of a card having been lost, the insuree shall report the occurrence of an insurance event to the insurer in accordance with paragraphs 1 and 2.
In the event that the loss of a payment card has resulted in the unauthorised use of the card, the “Notification of an Insurance Event” form must be accompanied by a copy of the police protocol on the loss of the payment card, or a sworn statement of the insuree. This entails a copy of a report on the theft of a payment card made by the police, or the submission of a complaint involving an unknown perpetrator in the case of the loss of a payment card.
In the event of the occurrence of an insurance event caused by the loss of personal documents or keys along with the payment card, in addition to the "Notification of an Insurance Event" form and the copy of a police protocol on the loss of the payment card, personal documents, and keys, it is also necessary to submit documentation on the payment of administrative fees for the issue of new personal documents, as well as sales slips for the procurement of new photographs for such personal documents and/or documentation of the payment of costs for reissuing keys or changing locks on the entryways to the insuree's place of permanent residence or recreational building of which the insuree or a person close to them is the owner.
If cash has been stolen within two hours of its having been withdrawn from an ATM machine or bank cash desk, in addition to the "Notification of an Insurance Event" form and the copy of a police protocol confirming that the police have been notified of the cash having been robbed, a copy of the receipt from the ATM machine or a copy of the bank statement must also be submitted.
Insurance events are settled only if the insurer has received all required documents. If the cardholder has failed to provide all of the required documents they shall have no claim to compensation for damages.
All compensation for damages provided from the insurance are paid to the account for which the payment card has been issued and in the currency of the account. Compensation for damages is provided up to the amount of the insurance amount. The amount of indemnity is stipulated by the type of insurance arranged.
