Terms of use

Any natural person (minor) who has a CSOB Children's Account and who has signed an Insurance Application.

How to proceed in the event of an insurance event?

insurance events may be announced using the form Insured Injury – insurance event report (57 kB). The form is also available on the Internet (to be printed out) or in the offices and branches of CSOB Pojistovna,
fill out (either you or your legal representative) the first and second page of the printout and give it to the doctor treating the insuree to be completed. If an operation has been made as a result of an accident, also provide a copy of the findings from the operation,
send the completed form to the address CSOB Pojistovna, a.s., Masarykovo Namesti 1458, 532 18 Pardubice, or deliver it to the nearest branch or office of CSOB Pojistovna.
